Recognizing anaphylaxis
Anaphylaxis is a rapid, severe, systemic allergic reaction. Signs: hives and flushing, swelling of the lips/tongue/throat, wheezing and stridor, difficulty breathing, and hypotension. The greatest threats are airway swelling and distributive (anaphylactic) shock.
التأق تفاعل تحسسي جهازي سريع شديد. علاماته: شرى واحمرار، وتورّم الشفاه/اللسان/الحلق، وأزيز وصرير، وصعوبة تنفس، وهبوط ضغط. وأكبر التهديدات تورّم المجرى الهوائي والصدمة التأقية (التوزيعية).
Epinephrine is the first-line, life-saving drug — give it intramuscularly (into the outer thigh) immediately. Do not delay for antihistamines or steroids; those are secondary. Maintain the airway, give oxygen and IV fluids, and be ready to repeat epinephrine.
الإبينفرين هو الدواء الأول المنقذ للحياة — أعطِه عضليًا (في الفخذ الخارجي) فورًا. لا تؤخّر لمضادات الهيستامين أو الستيرويدات؛ فهي ثانوية. حافظ على المجرى، أعطِ أكسجينًا وسوائل وريدية، وكن مستعدًا لتكرار الإبينفرين.
Auto-injector teaching
Teach patients with severe allergies to carry an epinephrine auto-injector, inject into the outer thigh (through clothing if needed), hold it in place for several seconds, and call emergency services and go to hospital even if they feel better — a biphasic (second) reaction can occur hours later.
علّم مرضى الحساسية الشديدة حمل حاقن إبينفرين ذاتي، والحقن في الفخذ الخارجي (عبر الملابس إن لزم)، وتثبيته عدة ثوانٍ، والاتصال بالطوارئ والذهاب للمستشفى حتى لو تحسّنوا — فقد يحدث تفاعل ثنائي الطور (ثانٍ) بعد ساعات.
