Memory trick

APGAR at 1 and 5 minutes, each item 0–2: Appearance (color), Pulse (heart rate), Grimace (reflex irritability), Activity (muscle tone), Respirations. Score 7–10 = good; 4–6 = moderate difficulty; 0–3 = needs resuscitation.

Normal newborn reflexes include Moro (startle), rooting, sucking, palmar/plantar grasp, and Babinski (toes fan out — normal in infants). Their absence, or persistence beyond the expected age, can signal a neurological problem.

At 1 minute, a newborn has a heart rate of 130, some flexion, a grimace to suction, a pink body with blue extremities, and a good cry. What is the APGAR?

Pulse >100 = 2, some flexion = 1, grimace = 1, body pink/extremities blue (acrocyanosis) = 1, good cry = 2 → APGAR 7. Acrocyanosis (blue hands/feet) is normal in the first hours and does not lower the score below expected.
